2083 Safety of Accelerated Schedules of Cluster Allergen Immunotherapy with House Dust Mites in Sixty Five Patients with Perennial BR.Asthma

      Background: To achive maintinance dose of House Dust Mites by cluster allergen immunotherapy in perennial allergic BR asthma

Methods: cluster allergen immunotherapy with 4-5 allergen doses of House Dust Mites ( HDM) of variable concentration at interval of one hour in 12hrs observation period

Results: in 20 %developed systemic reaction & in 80 % it is safe and well tolrated to achive the maintinence dose of allergen extracts of House Dust Mites ( HDM)

Conclusions: allergen extract of house dust mites(HDM) by cluster immunotherapy  is safe and have better compliance

introduction:The House Dust Mites is a major cause of perennial allergic BR ASTHMA.Allergen immunotherapy is the only therapy which can modify the natural course of disease .The success of allergen immunotherapy is dose- dependant as well the quality of extract used and compliance by the patient. The conventional subcutaneous immunotherapy(SCIT) is a slow treatment that often leads to poor compliance or discontinuation of treatment

 The safety of cluster allergen immunotherapy has been studies in sixty five patients of perennial allergic bronchial asthma. Maximum maintenance dose of House Dust Mites ( HDM ) extracts was administered to patients by subcutaneous injection with designed protocol of  Cluster immunotherapy  as per maximum tolerance of patients with variable concentration of maintainence dose at  interval  of one hr in a 12hr observation period and subsequent doses prescribed at minimum duration from 5-7 days to achieve maximum tolerence dose  (MTD) or optimum dose in 6-8 weeks.All 65 patients of perennial bronchial asthma received the planned maximal dose of House dust mite extracts of D-farinae  & D-pteronyssinus . Systemic reaction was found in 20 % of the patient which was well controlled after administration of  injection adrenaline .No severe or near fatal reactions was found  in 80 % of the patients after administration of maximum tolerence dose (MTD) in duration of six weeks . Cluster allergen immunotherapy (CIT) is a useful therapeutic option , safe and well tolerated by  our patients in perennial allergic bronchial asthma who are,immunologically significant  Sensitive to House Dust Mites .
