1124 Health care transition: Allergic children and adolescent perspectives of health care transition in rio de janeiro state university , Brazil

Background: A better understanding of the needs of children and adolescent with special care needs like allergic diseases is essential to provide health care transition service. Mainly when we are in public healt services. Department of Pediatric in Rio de Janeiro Universit is creating this service. The purpose of this study was to evaluate the risk profile of children with chronic disease like allergic children. 

Methods: This was a cross-sectional study based on primary data collected from direct consultation of patient charts. Included patient with 10 to 12 years attended in ambulatory care in the year of 2013.
 

Results: Mean age was 11 years±4.6 months and 57% was female. From 280 patients 48% have allergic disease. Riniths 65%, Asthma 38% and 39 both. The 40% of allergic patients were not being medically. Among asthmatic group, 25% did not provide data about chronic complications. The most common comorbid was obesity 30%.

Conclusions: The process of transition from child to adult in public health is complex  The close relationship between the health care professional staff and children help to create a safe transition. More research that considers this important moment is need.
