Background: Share the presentation of an unusual case of sinusitis and bronchiectasis.

Methods: Clinical assessment, imaging( CT scan of PNS and Chest, ultrasound abdomen, echocardiogram,chest x-ray), spirometry, saccharin dye test, semen analysis.

Results: Situs inversus totalis, absence of frontal & sphenoidal sinuses, oligospermia with dysmotile spermatozoa, cystic bronchiectasis in the superior segment of right lower lobe and tree in bud appearance in medial & lateral segments of left lower lobe, Delayed Saccharin test.

Conclusions: Kartagener's syndrome has a reported prevalence of one in 32,000. The present unmarried patient reported with symptoms of recurrent upper and lower respiratory tract infections and well known features with confirmed multisystem involvement on imaging and laboratory investigations. Counselled on long term management of the disorder and life style issues.
